SUBACUTE REHABILITATION
Our subacute rehabilitation program is designed for individuals who have been discharged from the hospital but require short-term rehabilitation in order to return home. With unique training protocols in kitchen, bedroom and bathroom settings, our patients work with a team of specialists to regain the skills and stamina necessary to return back into the community safely and independently.
We treat:
Cardiovascular, Diabetes, Neuromuscular, Palliative Care, Pulmonary, Respite Care, Stroke (CVA), Wounds and more.
LONG-TERM CARE
Sunset’s long-term care services provide family members with peace of mind knowing that their loved one’s daily needs are being met around the clock. Residents receive comprehensive treatment from a full team of health care providers, including physicians, nurses, certified nursing assistants, therapists, nutritionists and more. The team focuses on providing high quality hands-on care and assistance with activities of daily living. A special focus is given to retaining residents’ overall function while ensuring dignity and quality of life.
SECURED MEMORY CARE UNIT
Sunset also features a dedicated memory care unit for those with Alzheimer’s disease and other forms of dementia. Our empathetic caregivers have been hand-selected for their ability to deliver compassionate care while fostering self-esteem, happiness and security for each of our residents. The unit features visual cues, secured entrances and exits, specialized lighting, calming colors and more.
RESPITE/SHORT-TERM CARE
Sunset offers temporary short-term care for a loved one while you vacation, travel or tend to business. This program provides a safe option for your loved one and peace of mind for you!
