Notre Dame Health and Rehabilitation Center is a short-term and long-term care and assisted living community on 5 landscaped acres. The Norwalk, Connecticut, campus isn’t far from the shops and restaurants in the downtown area, with the Long Island Sound just a short drive away. Acute care is available at nearby West Medical and Norwalk Hospital.
Notre Dame Health and Rehabilitation Center has skilled nursing staff on-site 24 hours a day, including certified nursing assistants, licensed practical nurses and registered nurses. The community also offers intensive post-stroke neurological rehabilitation designed to help individuals return home as quickly as possible. Additional services include short-term rehab and hospice. The center also has a dementia and memory care unit with a Wanderguard system to keep residents safe.

This homelike community can accommodate up to 60 residents and includes private and semiprivate rooms. Residents can receive services from visiting beauty care providers and attend regular Mass at the on-site chapel. They share meals in the communal dining room and have the opportunity to take part in social events and activities.
