Crossroads at Lakewood welcomes residents into a beautiful Craftsman-style building with large windows, comfortable furnishings, and plenty of amenities to care for your loved one. Residents can stay in a private studio apartment or share an apartment with a roommate.
The space is designed specifically for those with dementia, and residents will be allowed to explore the community safely. We also have a specialized memory care program called “In the Moment,” which focuses on physical engagement, spiritual support, artistic expression, community connections, continuing education, and lifestyle and leisure.

- Routine assistance with daily living activities, such as bathing, dressing, and hygiene

Support Services
Our residents receive a comprehensive health assessment to determine the type and amount of care and social services needed, including the following:
- Coordination of care with home and hospice providers
- Rounding physicians who visit residents regularly in the privacy of his or her apartment
- Resident-specific activity programming, including in-house activities, group activities, and outings
- Therapeutic diets
- There may be an additional fee for complex care needs, which will be determined during the assessment.
Respite Care
Respite care: $250 per day
Our community provides an option for those needing memory support services. You may consider using respite services for family vacations, caregiver unavailability, or trial stays. Daily fees for respite care are collected in advance and range from 14-30 days.
