rehabilitation in The Assisi Health Care Center.

Center at Clare Oaks, featuring assisted living, specialty care for memory
support, skilled nursing and rehabilitation in a warm, caring environment with
unparalleled service and attention to detail. You do not have to be a Clare Oaks
resident and admissions are welcome 24 hours a day, 7 days a week. The Assisi
Health Care Center is Medicare certified and offers private rooms with private
bath.

respite care are all available.
Here, residents enjoy the best of home and much more, knowing friendly staff
